ГЕМОСТАТИЧЕСКИЕ МАТЕРИАЛЫ НА ОСНОВЕ ХИТОЗАНА И КОЛЛАГЕНА, ПРИМЕНЯЕМЫЕ В ХИРУРГИЧЕСКОЙ ПРАКТИКЕ
Аннотация
Неконтролируемое кровотечение — ведущая причина предотвратимой смертности в хирургической практике. Это обусловливает необходимость разработки эффективных и безопасных местных гемостатических материалов. Наибольший интерес представляют хитозан и коллаген, обладающие доказанной биосовместимостью, биоразлагаемостью, а также способностью активировать механизмы гемостаза и тканевой регенерации.
Цель исследования — систематизация и анализ данных о разработке, эффективности, клиническом применении и перспективах использования хитозан- и коллагенсодержащих гемостатиков в хирургии.
Материалы и методы. Проведен аналитический обзор публикаций в базах данных РИНЦ, Scopus и PubMed за последние 3 года. Стратегия поиска включала русскоязычные ключевые слова («местные гемостатические материалы», «интраоперационное кровотечение», «хитозан», «коллаген») и англоязычную комбинацию с логическими операторами: (chitosan OR collagen) AND (hemostasis OR hemostatic OR hemorrhage) AND (surgery OR surgical). Качество включенных клинических исследований оценивали по шкале MINORS, а риск систематической ошибки в экспериментальных работах — с помощью инструмента SYRCLE RoB.
Результаты. Установлено, что коллаген инициирует физиологический гемостаз и стимулирует регенерацию, тогда как гемостатический эффект хитозана реализуется независимо от функционального состояния коагуляционной системы крови. Создание композитных материалов (включая хитозан-коллагеновые матрицы, модификацию полифосфатами и β-глюканами) обеспечивает синергизм гемостатического, антимикробного и регенераторного действия.
Заключение. Композитные материалы на основе хитозана и коллагена представляют собой перспективное направление в разработке местных гемостатических средств нового поколения.
