СОВРЕМЕННОЕ ПРЕДСТАВЛЕНИЕ ОБ ИСПОЛЬЗОВАНИИ ИМПЛАНТАТОВ НА ОСНОВЕ ПОРИСТОГО ТИТАНА И ЕГО СПЛАВОВ ДЛЯ ЗАМЕЩЕНИЯ КОСТНЫХ ДЕФЕКТОВ
Аннотация
Титан в качестве материала для замещения дефектов кости нашел широкое применение в травматологии и ортопедии. Изучение различных сплавов титана, изменение структуры имплантата и модифицирование его поверхности позволяет быть биоэквивалентным к костной ткани.
Цель — предоставить полноценную картину об использовании титанового аугмента в качестве остеозамещающего материала, проанализировав современные литературные данные.
Материалы и методы. Произведен анализ отечественных и зарубежных статей, авторефератов и диссертаций с помощью электронных баз данных PubMed, Google Scholar, eLibrary, Science Research Portal, Cochrane Database за период 2000-2022 гг.
Результаты и обсуждение. Представлены сравнительные биомеханические характеристики металлических имплантатов по отношению к костной ткани. Выделена классификация титановых аугментов. Определен оптимальный размер пор и степень пористости в титановом аугменте для остеоинтеграции. Приведены данные о повышении остеоинтеграции путем модифицирования поверхности имплантата, добавления различных факторов роста и мезенхимальных стволовых клеток. Репрезентированы результаты пластики костных дефектов с помощью металлических имплантатов из сплава титана.
Выводы. Титан является ценным материалом для аугментации костных дефектов за счет своих биологических, остеокондуктивных, механических и прочностных свойств, способен к полной остеоинтеграции с подлежащей костной тканью. Высокопористый титановый имплантат, полученный методом 3D-печати, является перспективным материалом для замещения импрессионных дефектов при внутрисуставных переломах. Приоритетными направлениями становятся клинические и экспериментальные исследования аугментов из композиционных материалов на составе титановой матрицы. Проблема модифицирования поверхности пористых металлических имплантатов с возможностью контролированной остеоиндукции и остеоинтеграции, а также приданием антибактериальных свойств Ме-имплантату, остается открытой для изучения в будущем.
